Baby Care Kit - Kits provides EOCCO parents with resources and supplies

Thank you for your interest in distributing the EOCCO Baby Care Kit in your community! The kit provides EOCCO parents with resources and supplies they would not typically receive when leaving the hospital after giving birth. These resources will help parents make informed decisions for their babies and themselves. The kit includes:

Currently, kits are available in Gilliam, Grant, Malheur, Union and Wallowa counties and have done well as a supplemental resource to existing programs via Local Community Health Partnerships (LCHP) and community-based organizations (e.g., Families First and Building Healthy Families).

Tracking Form

The EOCCO asks that you also help track the following information on the kits your organization distributes:

  • Total number of kits distributed
  • Identify the county you distributed the kits
  • EOCCO IDs of the members who received the kits (to the best of your ability)

EOCCO will request the above information annually via this secure linkas long as your organization continues to participate. You can submit on a cadence that works best for your organization as long as all data is logged by December 31st of the reporting year. Providing this information will help EOCCO evaluate the program and apply these kits to our health-related services funds.
