- If they already have an assessment from the last 30 days, they do not need to schedule a visit with their PCP to get an assessment.
- If their provider asks for a template to fill out for their assessment, they can print out and bring this blank form for the provider to fill out. [insert nutrition assessment form PDF]
- For medically tailored meals only: Help the member schedule their visit with the registered dietician. You can also help the member request a copy of their assessment once it is completed.
- Fill out the online (HRSN) request form.
- EOCCO will review their request and let them know by email or mail if they are approved or denied.
- If the member is approved, EOCCO will assign them to an HRSN service provider to begin providing them with their approved service.
