ORPRN Technical Assistance Opportunity for the New Social Determinants of Health (SDoH): Social Needs Screening and Referral Incentive Measure 

In January, a new incentive measure, SDoH: Social Needs Screening and Referral, will be rolling out across CCOs. This incentive measure recognizes the profound impact that social, environmental, and systemic factors have on a person’s health. It aims to advance health equity by creating a transformative healthcare delivery system that ensures people’s social needs are acknowledged and addressed.

2023 is a self-assessment measurement year for CCOs. Starting in 2024, CCOs will report on the percent of members who received a social needs screening and, if appropriate, were referred to services in the domains of: housing insecurity, food insecurity, and transportation

In preparation for 2024, Oregon Rural Practice-based Research Network (ORPRN) is offering a free individualized support opportunity to clinics in Oregon! ORPRN will meet clinics where they’re at in the social needs screening process, and work with them to develop and implement a social needs screening related quality improvement intervention.

This program will begin in early 2023 (after recruitment) and conclude in June of 2023. Participation in the program involves:

  • Completing a brief survey at the time of recruitment.
  • 2 webinars: Learn about a variety of SDoH screening topics and hear from clinical peers regarding social needs screening implementation and screening data usage. 
  • Technical assistance: 12 hours of one on one support tailored to your clinic’s needs and goals. This will include support from an EHR specialist.
  • Pilot testing a small social needs screening quality improvement intervention of your choosing. Share results with ORPRN.

This ECHO will be held via videoconference on 1st and 3rd Thursdays from noon -1 p.m., from March 23 - June 1, 2023.

2023 EOCCO Calendar Year Timeline for Clinics Now Available

EOCCO has recently updated their 2023 Calendar Timeline with deliverables and events occurring throughout 2023.  This includes reporting due dates for deliverables such as Behavioral Health Integration, Language Access Reports, and yearend data submissions, as well as annual in-person trainings, Community Benefit Investment Reinvestment (CBIR) grant cycles and payment distributions.  To view the full calendar of events, visit: EOCCO Calendar Year Timeline 2023.

Updates to Therapy Authorizations for 2023

Beginning in 2023, EOCCO will remove the December 31 end date for all therapies but will remain on a calendar year for visit counts. All therapies will be loaded for a 6-month time frame, even if it spans into the following year. The therapy visit counts will remain on a calendar year, so whatever year the start date the auth falls under is the year the visit count will fall under. For example, a request for 12 PT visits starting 12/02022 will be approved for date span of 12/01/2022- 06/01/2024 and all 12 visits would apply to the 2022 year.

Upcoming Events and Trainings:

Motivational interviewing trainings – Level 2 and Group Trainings Now Available (no-cost CME available)

Oregon Medicaid providers are invited to no-cost virtual trainings (with no-cost continuing medical education credits) on motivational interviewing. The course will use diabetes management as the core topic during the training.

The trainings will focus on improving providers' confidence and skills in conversations about sensitive behavior change topics. Dana Sturtevant, MS, RD, will lead these trainings. Three types of trainings are available, with multiple opportunities to attend each between June 2022 and February 2023:

Gender-Affirming Hormone Therapy (GAHT) Community of Practice

This twice-monthly community of practice will support primary care providers in offering gender-affirming hormone therapy (GAHT) to their ADULT* patients.
The first three didactic sessions will cover the process of hormone initiation, making referrals to adjacent gender-affirming care, and management and surveillance of testosterone- and estrogen-centered therapies.

The remaining sessions will provide key clinical takeaways, followed by case-based discussion using participants’ cases, related to topics such as lab conundrums, nuances in hormone formulation adjustment and titration, best practices for informed consent practices, and more.

Sessions take place via Zoom on the 2nd and 4th Wednesday of each month, 12pm - 1pm PST,
January 11th - June 14th, 2023

Care Delivery Area Reminder

As we approach the end of the year, we wanted to remind clinics about the two new Care Delivery Areas (CDA) implemented as part of the 2022 Shared Savings model. These areas align with the goal to transform health care and encourage higher quality and more efficient delivery of Covered Services to Member, aligning with the overall EOCCO Value Based Payment model structure and goals. The two 2022 Care Delivery Areas are:

  • Maternity Care Delivery Area: The performance measure is Postpartum Care
  • Hospital Care Delivery Area: The performance measure is Plan All-Cause Inpatient Readmits

Colorectal Cancer Screening for Rural Communities ECHO

The Oregon ECHO Network is excited to offer the Colorectal Cancer (CRC) Screening Outreach for Rural Communities ECHO program this coming spring. This 6-session ECHO will share evidenced-based approaches for clinical practices or regional health care organizations to improve colorectal cancer screening rates.

The sessions will cover topics such as how to identify patients for screening, implementing a program that fits local needs and unique barriers, and providing patient navigation throughout the screening process. They are recruiting program champions from health care or community organizations (health plans, health systems, or clinical practices) interested in improving CRC screening rates using population outreach.

Strive Health

Coming Soon! We are excited to announce that EOCCO is partnering with Strive Health to offer enhanced care services to our patients with kidney disease. This value-based care model supports us providing high-touch, wraparound care for patients with this complex disease.

Who does Strive Health serve? Patients with Chronic Kidney Disease (CKD 3, 4 & 5) and End-Stage Renal Disease (ESRD) and who have Medicaid. The team specializes in late-stage kidney disease but also supports other disease and care management patient needs.

How this program works: The Strive Health team is comprised of nurse practitioners, RN care managers, social workers, dietitians, and care coordinators that collaborate to provide disease management, co-morbidity management, and wraparound case management along with education, nutrition, and SDOH support. This team serves as an extension of your practice to offer additional care and resources to your patients. They will also closely collaborate with each patient’s nephrologist and other specialists to ensure care plan understanding and compliance.

What does this mean for my practice? The Strive Health team will work with providers to coordinate additional support to patients that qualify. The team requests your collaboration to engage patients into the program and more information will be provided in the future on how to refer them. The team will also send after appointment visit notes, so you are aware of the support that was provided.

As the program launches, a representative from Strive Health will be reaching out to better understand how to best support your eligible patients. Many of you attended Strive Health implementation calls during the launch of the Summit Health partnership, but if you did not have an opportunity, or if you wish to attend again, Strive Health will offer informational meetings to review the program.
