Long-Acting Insulin Formulary Change

Effective 04/01/22, EOCCO will remove Basaglar (insulin glargine) from the formulary and add Insulin Glargine (insulin glargine-yfgn). This is an FDA-approved biosimilar insulin product for patients with type 1 or type 2 diabetes mellitus, interchangeable with Lantus. Insulin Glargine (insulin glargine-yfgn) is available in both a 10ml vial and a 3ml pen, providing your patient the same strength and type of insulin as Basaglar.

What this means for EOCCO members:

Starting 04/01/22, prescriptions for Insulin Glargine (insulin glargine-yfgn) will approve at the pharmacy without a prior authorization if filled within existing quantity limits (60ml per 30 days). In anticipation of the formulary change, please send a new prescription for Insulin Glargine (insulin glargine-yfgn) to your patient’s pharmacy. A new prescription template can be found here.

Prescriptions for any other long-acting insulin, or for Insulin Glargine (insulin glargine-yfgn) above 60ml per 30 days, will require a prior authorization. If Insulin Glargine (insulin glargine-yfgn) is not appropriate, please submit a prior authorization on behalf of the patient documenting contraindication or intolerance to Insulin Glargine (insulin glargine-yfgn).

Clinical Quality Measure Data Submission

All data for the EHR-based CCO Clinical Quality Measures (CQMs) is due to EOCCO by Friday March 4th. Please contact EOCCOmetrics@modahealth.com as soon as possible if your clinic plans to report on any of the following measures for the 2021 measurement year and you have not yet received a Data Submission Template:

  • Depression Screening
  • Diabetes HbA1c Poor Control
  • Controlling High Blood Pressure
  • Cigarette Smoking Prevalence
  • Drug and Alcohol Screening (SBIRT)

Please return your Data Submission to EOCCOmetrics@modahealth.com by Friday March 4th. Don’t hesitate to reach out via email or phone with any questions or concerns.

Q4 2021 Language Access Report Due 3/23

The quarterly Language Access Report is due to EOCCO by 3/23. This report will include information on all member encounters that occurred between October 1, 2021 - December 31, 2021 and required interpretation. Interpretation provided by a bilingual staff member or phone, video, or in-person services provided by a contracted language service provider should be included in the report. For more information on the reporting template visit Language Access reporting template available on the EOCCO website.

Clinics can also request that EOCCO compile a prefilled template to assist in the reporting process. This report will use claims data to pull member information and the date of service that the visit occurred. Clinics will need to complete the report by filling in any interpreter information that was provided using their EHR or language service provider invoices. Please note that your clinic may have provided services to additional members who were not listed on the report. To request a prefilled template or to submit a report please email EOCCOmetrics@modahealth.com

Upcoming Events & Trainings

Virtual Learning Collaborative: Care Coordination and Intensive Care

The Oregon Health Authority Transformation Center is hosting a monthly virtual learning collaborative to support CCOs and other organizations that provide care coordination and intensive care coordination (CC/ICC) services to Oregon Health Plan members. This will include topics such as CCO contract requirements and OARs related to CC/ICC, reporting requirements, and best practices for interdisciplinary care team (ICT) meetings, sharing assessments and care plans, and CC/ICC staffing models.

Webinar: Improving HPV Vaccination in your Health System through Quality Improvement

Review evidence-based strategies to improve HPV vaccination rates, improve your ability to make HPV vaccination recommendations, and learn about resources for clinics. This webinar is hosted by the Oregon Pacific Area Health Education Center and Samaritan Health Services. CME credits are available.

Spring ECHO Program Registration Launch

ECHO is an interactive, virtual learning collaborative designed to help primary care clinicians and teams better care for patients with complex conditions or improve their systems of care. All programs are free and offer no-cost continuing medical education credits. The Oregon ECHO Network is housed at OHSU within the ORPRN Research Center.

Spring ECHO offerings include programs in Diabetes Care for Whole-Person Health, Partnering with Community Health Workers, and Child Psychiatry. To learn more and see the whole roster of upcoming programs, go to https://www.oregonechonetwork.org/

Potential Records Review Request

Beginning April 2022, the Oregon Health Authority and Health Services Advisory Group (HSAG) will be conducting a Medical Record Review of a portion of EOCCO members randomly pre-selected by HSAG. Your clinic/facility may be contacted by EOCCO to ask to procure a medical record for this review if your patient falls into the randomly pre-selected batch

Potential members that could be included in this review are those that have been continuously enrolled at EOCCO from July 1, 2020 to June 30, 2021, and have had at least one physician visit during that period of time. Additionally, members with Medicare and/or other insurance coverage will be excluded from the eligible population since these members may have received services that were documented in their medical record but are not represented in OHA’s encounter data.
