This year the Oregon Health Authority (OHA) implemented a new statewide incentive measure called Social Determinants of Health (SDoH): Social Needs Screening and Referral. This incentive measure recognizes the profound impact that social, environmental, and systemic factors have on a person’s health. It aims to advance health equity be creating a transformative healthcare delivery system that ensure members’ social needs are acknowledged and addressed.
Starting in 2023, the Social Needs Screening and Referral Measure requires an annual CCO self-assessment of social needs screening policies, resources/services, and screening tools/questions used by contracted clinics and community organizations. In 2025, CCOs will begin reporting on the percent of members who were screened for social needs (including housing, food, and non-medical transportation needs) and, if appropriate, were referred to services.
To help us better understand how and where Eastern Oregon CCO (EOCCO) members are being screened for social needs, we are asking all contracted providers, clinics, and organizations to complete the Social Needs Screening Survey. Please complete this survey by Friday, September 8th, 2023.
This survey asks respondents to share information on:
- If patients or clients are being screened for social needs
- Social needs screening tools or questions used
- The frequency at which patients/ clients are screened for social needs
- How screening results are captured and shared
- Any barriers to conducting social needs screenings
