HRSN nutrition benefits
Good nutrition plays a big role in your health. If you’re having trouble preparing meals that support your health, EOCCO may be able to help.
If you are eligible, you may qualify for free meals delivered to your home or for one-on-one or group nutrition education at no cost to you through our Health-Related Social Needs (HRSN) benefit.
What nutrition services are covered?
Medically Tailored Meals (MTM)
Get healthy meals delivered to your home, based on your medical needs. These meals are created by a registered dietitian and made to support your health conditions.
Examples of medically tailored meals:
- Diabetes-friendly
- Heart-friendly
- Gluten-free
- High protein
- Lower sodium
- Vegetarian
- Kidney-friend (renal)
- Pureed meals
You may qualify for up to three meals a day for six months.
Not included:
- Meals if you live in a place that already provides food
- If you do not have access to a working refrigerator
Nutrition education
Learn more about how food affects your health through one-on-one or group sessions. You may qualify for up to three hours per week.
Not included if you:
- Already receive Medical Nutrition Therapy (MNT) through your regular health benefits
Fruit and vegetables (you can apply starting July 1, 2026)
A grocery card to shop for fruits and vegetables at your local grocery store or fruit and vegetable boxes sent to you weekly.
Examples of fruits and vegetables available:
- Fresh fruit or vegetables
- Canned or packaged fruits such as applesauce
- Frozen or dried fruits without added sugars
- Fresh herbs
You may qualify for up to 24 weeks of a fruit and vegetable grocery card or box delivery.
Note: Once you select your delivery option (card or box) you cannot change it.
You are not eligible if:
- You live in a place that already provides food
- You are already receiving the Medically Tailored Meals or Pantry Stocking benefit
Pantry Stocking (you can apply starting July 1, 2026)
A grocery card to shop at your local grocery store or pantry stocking boxes sent to you weekly.
Examples of foods available:
- Fruits and vegetables
- Meat, poultry, fish, and other proteins
- Legumes
- Dairy products
- Breads and cereals
- Cooking oils, spices, and herbs
- Snack foods and non-alcoholic beverages
- Seeds and plants that produce food for the household
You may qualify for up to 24 weeks of a pantry stocking grocery card or box delivery.
Note: Once you select your delivery option (card or box) you cannot change it.
You are not eligible if:
- You live in a place that already provides food
- You are already receiving the Fruit and Vegetable or Medically Tailored Meals benefit
Note: You can only get Medically Tailored Meals, fruit and vegetable boxes, and pantry stocking benefits one at a time.
Who can get nutrition help?
You may qualify if you:
- Are an EOCCO member with Oregon Health Plan (OHP) medical coverage under a CCOA or CCOB plan
- Have trouble making your own meals or getting enough food to eat (low or very low food security)
- Have a medical condition that would be improved by nutrition support
- Meet at least one of these life situations:
- You’ll be starting Medicare and OHP in the next three months
- You just joined Medicare and OHP in the last nine months
- You may lose your housing soon or spend most of your income on rent
- You got care at Oregon State Hospital or a large treatment center in the last year
- You were released from jail or prison in the last year
- You’ve been in foster care or the child welfare system in Oregon
- You are a young adult with special healthcare need
- Service specific eligibility:
- Medically Tailored Meals
- Live somewhere that doesn’t provide meals
- Have access to refrigerator
- Have a registered dietitian assessment showing the need
- Have a qualifying health condition for which medically tailored meals are appropriate
- Nutrition Education
- Cannot receive this through your medical benefit
- Have at least one of the qualifying health conditions
- Complex physical health condition
- Complex behavioral condition
- Developmental or intellectual disability
- Difficulty with self-care and daily activities
- Experience of abuse or neglect
- Age 65 or older
- Under the age of 6
- Pregnant or gave birth in the last 12 months
- Use emergency room or crisis services often
- Fruits and vegetables
- Live somewhere that doesn’t provide meals
- Have a qualifying health condition:
- Complex physical health condition
- Complex behavioral condition
- Developmental or intellectual disability
- Difficulty with self-care and daily activities
- Experience of abuse or neglect
- Age 65 or older
- Under the age of 6
- Pregnant or gave birth in the last 12 months
- Use emergency room or crisis services often
- Pantry stocking
- Live somewhere that doesn’t provide meals
- Have a qualifying health condition:
- Currently pregnant
- 5 years old or younger
- Child or young adult aged 6-20 with one or more of the following health conditions:
- Complex physical health condition
- Complex behavioral health condition
- Developmental or intellectual disability
- Difficulty with self-care and daily activities
- Experience of abuse or neglect
- Use emergency room or crisis services often
You may still qualify if you are already getting WIC, SNAP or other nutrition assistance.
How to request help with nutrition
- Make sure you qualify (check the list above)
- For medically tailored meals only: Schedule a visit with your primary care provider (PCP) and ask for a referral to a registered dietitian
- If you already have an assessment from the last 30 days, you do not need to schedule a visit with your PCP to get an assessment.
- If your provider asks for a template to fill out for your assessment, you can print out and bring this blank form for them to fill out. [insert nutrition assessment form PDF]
- For medically tailored meals only: Complete an assessment with the dietitian. (Remember to get a copy!)
- For Fruit & Vegetables or Pantry Stocking card/voucher requests only: see if there is a grocery store accepting HRSN cards/vouchers in your area.
- Fill out the online HRSN request form. Tip: Your doctor, dietitian, traditional health worker or other support person can help you fill this out
- Submit your request, including your dietitian’s assessment if you’re requesting medically tailored meals
- EOCCO will review your request and let you know by email or mail if you’re approved
- If you are approved EOCCO will assign you to an HRSN service provider to begin providing you with Medically Tailored Meals.
- If your HRSN request is denied by EOCCO, you sill get a letter explaining why.
If you are approved for medically tailored meals, EOCCO will send your referral directly to begin delivery.
Requirements for Fruit and Vegetable, Pantry Stocking, and Medically Tailored meal box delivery:
- A home delivery address matching what is in OHP’s system.
