Livongo Diabetes Self-Management Program

EOCCO has recently partnered with Livongo, a comprehensive diabetes self-management program offered to EOCCO members with type I or type II diabetes. This digital program uses an app, a digital meter, and personal health coaching to help members manage their diabetes through data tracking, diabetes education, and customized feedback and support for each member. Once enrolled, members will receive unlimited strips and lancets delivered to their home.

Providers and care teams can refer a member to the program by visiting https://join.livongo.com/EOCCO/now. EOCCO will reach out to members with type I and type II diabetes beginning in April to share information about the benefit and encourage them to enroll.

Member PCP Assignment FAQ

An FAQ guide for PCP assignmentshas been uploaded to the EOCCO website.This guide includes questions around how an EOCCO member can initially select their PCP, how EOCCO assigns members who have not selected a PCP, and how a member or provider can request a PCP change.

As a reminder, primary care offices can request a PCP change on behalf of a member per the member’s request or if the member is solely receiving care at a clinic other than their assigned PCP.Requests can be made by completing theprimary care provider change form.

Well Child 3-6 Incentive Program

The Well Child Incentive program will continue into the 2022 metric year with a few modifications. This program allows for members 3-6 years old to receive a $50 Amazon gift card upon completion of their yearly well child visit. 

Flyers are available in English and Spanish and will be mailed directly to the parents of members turning 3-6 throughout the year (parents can expect to receive the flyer around the time of their child’s birthday). To qualify, visits do not need to occur after the member’s birthday but can occur anytime during the year that a member is turning 3, 4, 5, or 6 years old. For more information, or to request copies of the flyers sent to members, contact EOCCOmetrics@modahealth.com.

Connect Oregon Kickoff

EOCCO and Unite Us have been working closely together to expand the coordinated care network, Connect Oregon, into Eastern Oregon to connect health and social care providers to deliver integrated whole-person care.

You will have the opportunity to join the Connect Oregon network along with EOCCO and community partners. Through this coordinated care network, users can seamlessly connect patients to community network partners through electronic referrals and spend less time searching for the right service.

We are excited to invite you to join our Engagement Kickoff on January 25, 2022, 11am-12pm. This meeting is an opportunity to learn more about the Connect Oregon network and how this partnership will affect your team. Please RSVP here to confirm your attendance.

Tobacco cessation counseling training to address higher COVID-19 risk for cigarette smokers (with CME)

With cigarette smokers at higher risk for COVID-19,this short online course will improve your care team’s ability to help patients quit tobacco. The course focuses on brief tobacco intervention and motivational interviewing techniques.

The training is available at no cost. Participants can earn up to 1.0 CME.

The course is self-paced and takes approximately 45 minutes. The course can be started, paused and resumed later as needed.

OHA Website & Hotline for COVID-19 Positive Cases

The Oregon Health Authority (OHA) has a new website where members can complete an online survey to report their positive case. Members can also get information on isolation and quarantine plans, and ways to keep others safe while they recover.

Please visit,Oregon.gov/positivecovidtestto find the Case Investigation Survey. Members can also use the COVID-19 Case Support Hotline, 866-917-8881. They are open from 8 a.m. to 6 p.m. Monday through Friday, and 10 a.m. to 4 p.m. Saturday. Hotline staff can answer questions about isolation, quarantine, resources to help them during isolation and can give general health information.

EOCCO's 2022 Member Handbook and 2022 Provider Manual are now available on eocco.com.

Members and providers can visit their respective home pages to find these updated materials.

Applications Now Open for the OHSU Knight Cancer Institute Community Partnership Program

The OHSU Knight Cancer Institute Community Partnership Program is pleased to announce that a Tiered Grant funding cycle is now open. Applications will be accepted for projects that aim to address local, community-specific cancer-related needs in Oregon. In addition to funds, grantees gain access to trainings, technical assistance, and networking opportunities to enhance project success.

Grants are available in three tiers ($10,000, $25,000, and $50,000) to support organizations in taking an initial idea and moving through the funding tiers to develop a sustainable effort. The Knight Cancer Institute will be holding an RFP overview webinar on January 26, 2022 at 10 a.m. (to register visit the Applicant Resources page)  Full proposals are due March 10, 2022.
